Unfortunately this is not working on SymbianOS 9.1. Version 1.04 USB is for S60v2.

On the Warelex website the last version for OS 9.1 USB is 1.00.

I tried myself on Nokia E65 to install
Warelex.Mobiola.WebCam.USB.v1.04.S60v3.SymbianOS9. 1.Unsigned.BiNPDA and it didn't work. Version 1.00Symbian 9.1 from their site work, but for 5 minutes, then you must restart (trial)

still waiting for USB OS 9.1 !!!!
